Sole Proprietorship
✓ Washington State Business License
✓ Social Security Number of the proprietor OR,
✓ Employer Identification Number (EIN) for the business

✓ Washington State Business License
✓ Employer Identification Number (EIN) for the business
✓ Social Security Number (SSN), address, and identification of each account signer
✓ SSN, address, and copy of identification for all business owners

Limited Partnership/Limited Liability Partnership
✓ Secretary of State Registration
✓ Washington State Business License
✓ Employer Identification Number (EIN) for the business
✓ Social Security Number (SSN), address, and identification of each account signer

✓ Meeting Minutes signed by the secretary or acting secretary, which specifically list the names and titles of those authorized to transact business at Industrial Credit Union
✓ Employer Identification Number (EIN) for the business; if using EIN from parent company, then a Letter of Authority is required

✓ Letter of Authority
✓ Employer Identification Number (EIN) for parent organization
✓ 501c Form Granting Non-Profit Status or Secretary of State Certificate Showing Non-Profit Status
✓ Social Security Number (SSN), address, and identification of each account signer
✓ SSN, address, and copy of identification of any individuals not listed on the account who have significant control of the organization

Political Campaign
✓ Federal Tax Identification Number -Employer Identification Number (EIN) or Social Security Number (SSN)
✓ SSN, address and copy of identification of account signer (Treasurer of Committee)
✓ Registered and stamped Public Disclosure Commission (PDC) Form C1 or C3
✓ Committee name on PDC must match the name on the campaign account
✓ PDC form must list Industrial Credit Union
